Regulation Requirements: All candidates must be registered or willing to register with SSSC and PVG to ensure eligibility to work in care settings.

I acknowledge that if any of the information supplied by me is found to be falsely declared, it may constitute a fundamental breach of my contract, and my employment could be terminated immediately.

I understand that any offer of employment will be conditional upon:

1. A satisfactory response regarding my status on the Protection of Vulnerable Groups (PVG) register.
2. Receipt of two satisfactory references.

By signing this declaration, I authorise Amare HC Solutions to request a PVG background check as part of the recruitment process. I further undertake to notify my employer immediately of any changes to my PVG status, including but not limited to:

• Being charged with an offence.
• Referral to any register or barred list.
• Withdrawal of any registration required for my employment.
